How to order

To order an ultrasound, fax us an order to 980-206-0163 including: patient name, DOB, e-mail & number along with requested  exam CPT and ICD- 10 code,  any pertinent history related to test being ordered (lab values, Hx of surgeries, area of concern,
Dx to rule out, past imaging results, etc.). The order form in the word document below can be downloaded,
filled-in and faxed to us. Once received, we will reach out to the patient to get exam scheduled. 

You will receive the final report via fax from our board-certified radiologist within 48 business hours after exam completion.
